Kidney yīn vacuity
肾阴虚 〔腎陰虛〕shèn yīn xū

A disease pattern chiefly characterized by aching lumbus; seminal emission; scant menstruation; dizziness; tinnitus.
Description: Limp aching lumbus and knees; dizziness; tinnitus; loosening of the teeth; hair loss; in men, persistent erections, seminal emission, and premature ejaculation; in women, scant menstruation of amenorrhea or flooding and spotting; insomnia; forgetfulness; dry mouth and pharynx; emaciation; tidal heat effusion (steaming bone tidal heat); vexing heat in the five hearts; night sweating; postmeridian reddening of the cheeks; short voidings of yellow urine; a red tongue with little or no fur and little liquid. A pulse that is fine and rapid.
Diseases: Vacuity taxation; insomnia; dizziness; tinnitus; forgetfulness; heat effusion; sweating; persistent erection; seminal emission; dispersion-thirst.
Biomedical correspondence: Liver-kidney yīn vacuity is seen in some forms of hypertension, neurosis, and menstrual disorders, whereas heart-kidney yīn vacuity may occur in tachycardia, hyperthyroidism, and neurosis. Lung-kidney yīn vacuity is seen in pulmonary tuberculosis.
Pathogenesis: Insufficiency of kidney yīn as a result of any of the following factors:
- fire forming as a result of excesses among the seven affects;
- constitutional weakness or general debilitation attributable to advancing age;
- enduring sickness or taxation fatigue;
- sexual intemperance;
- heat in the advanced stages of febrile disease;
- excessive consumption of warm, dry
yáng-invigorating
medicinals (that is, items that treat impotence).
Analysis of signs
- Yīn vacuity and depletion of essence and marrow: Dizziness, tinnitus, forgetfulness, emaciation, loosening of the teeth, hair loss, and in limp aching lumbus and knees.
- Vacuity heat: Vexing heat in the five hearts, tidal heat effusion, steaming bone tidal heat; night sweating, and postmeridian reddening of the cheeks.
- Urine: Short voidings of yellow urine.
- Men: Depleted kidney yīn that fails to restrain kidney yáng can cause
frenetic stirring of the ministerial fire
(liver-kidney yīn vacuity with vacuity fire flaming upward), which manifests in excessive libido, frequent and persistent erections, seminal emission, and premature ejaculation. - Women: Scant menstruation or amenorrhea. When severe yīn vacuity stirring fire gives rise to frenetic movement of hot blood, there may be flooding and spotting.
- Tongue: Red with scant liquid and little or no fur.
- Pulse: Fine and rapid.
Treatment
Medicinal therapy: Treat by the principle of invigorating the governor of water to restrain the light of yáng.

Combined patterns
- Lung-kidney yīn vacuity, marked by dry cough with scant phlegm, hoarse voice, seminal emission, and menstrual irregularities, with vacuity heat signs. Lung yīn vacuity and kidney yīn vacuity are mutually conducive.
- Liver-kidney yīn vacuity, characterized by dizziness, tinnitus, limp aching lumbus and knees, rib-side pain, and insomnia with vacuity heat signs and signs of frenetic stirring of the ministerial fire. This pattern is often attributable to kidney disease affecting the liver (water failing to moisten wood).
- Noninteraction of the heart and kidney, characterized by heart vexation, heart palpitation, insomnia, limp aching lumbus and knees, seminal emission (dream mission), profuse dreaming, with yīn vacuity signs. This pattern can be attributable to kidney disease affecting the heart or heart disease affecting the kidney.
Further developments: Ascendant hyperactivity of liver yáng; dual vacuity of yīn and yáng with inner body vacuity heat and outer body vacuity cold.
