Internal damage stomach duct pain
内伤胃脘痛 〔內傷胃脘痛〕 nèi shāng wèi wǎn tòng
Pain in the stomach due to a variety of internal damage factors including accumulated cold, accumulated heat, spleen-stomach vacuity cold, yīn vacuity, food accumulation, phlegm-rheum, qì depression (qì stagnation), blood stasis, and worm accumulation.
Patterns
Accumulated cold (积冷 jī lěng) stomach duct pain is attributed to insufficiency of stomach yáng and damage by cold drinks causing yīn cold to congeal and bindand is characterized by stomach duct pain that refuses cold and is accompanied by counterflow cold of the extremities, uninhibited clear stool and urine (clean untransformed stool and clear urine), ejection of foamy drool, and a slow pulse.
Medicinal therapy: Warm the center and disperse cold with formulas such as
Acumoxatherapy: Base treatment mainly on alarm and uniting points of ST, PC, and SP. Main points: CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), PC-6 (Inner Pass, 内关 nèi guān), SP-4 (Yellow Emperor, 公孙 gōng sūn). CV-6 (Sea of Qì, 气海 qì hǎi) and ST-25 (Celestial Pivot, 天枢 tiān shū), needling with even supplementation and drainage and large amounts of moxa. Also, perform moxibustion on salt or ginger at CV-8 (Spirit Gate Tower, 神阙 shén què).
Accumulated heat (积热 jī rè) stomach duct pain is due to intense stomach heat or affect-mind binding depression gradually causing fire formation and is characterized by a urgent pain with scorching hot feeling in the stomach duct, thirst, dry lips, generalized heat and red face, constipation and copious sweating, rapid pulse, and in some cases vexation, agitation, and irascibilityand is treated by clearing heat and abduction with formulas like
Acumoxatherapy: Use the main points given above. For intense stomach heat, add ST-44 (Inner Court, 内庭 nèi tíng), ST-45 (Severe Mouth, 厉兑 lì duì), TB-6 (Branch Ditch, 支沟 zhī gōu), and PC-6 (Inner Pass, 内关 nèi guān), needling with drainage. For heat accumulation due to affect-mind binding depression transforming into heat, add LR-3 (Supreme Surge, 太冲 tài chōng), LR-14 (Cycle Gate, 期门 qī mén), GB-34 (Yáng Mound Spring, 阳陵泉 yáng líng quán), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), needling with drainage.
Spleen-stomach vacuity cold (脾胃虚寒 pí wèi xū hán) stomach duct pain is characterized by stomach pain that is worse on an empty stomach and slightly relieved by eating, warmth, and pressure, and accompanied by fatigued limbs and lack of strength, reduced food intake, sloppy stool, forceless fine sunken pulse.
Medicinal therapy: Supplement the spleen and warm yáng; use formulas such as
Acumoxatherapy: Base treatment mainly on back transport points, CV, ST, and SP. Main points: BL-20 (Spleen Transport, 脾俞 pí shù), BL-21 (Stomach Transport, 胃俞 wèi shù), CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), CV-6 (Sea of Qì, 气海 qì hǎi), LR-13 (Camphorwood Gate, 章门 zhāng mén), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), needling with supplementation and adding moxa. For spleen-kidney depletion and failure to contain blood, add BL-23 (Kidney Transport, 肾俞 shèn shù), CV-4 (Pass Head, 关元 guān yuán), KI-3 (Great Ravine, 太溪 tài xī), SP-10 (Sea of Blood, 血海 xuè hǎi), and PC-4 (Cleft Gate, 郄门 xī mén), needling with supplementation and adding moxa.
Yīn vacuity (阴虚 yīn xū) stomach duct pain is heat pain that is caused by yīn vacuity, is characterized by clamoring stomach and rapid hungering, dry mouth with little liquid, difficult defecation, red tongue fur (possibly with peeling fur), and a rapid fine stringlike pulseand can be treated with
Acumoxatherapy: Needle with even supplementation and drainage at main points given under spleen-stomach vacuity cold above. For yīn vacuity heat pain, add KI-6 (Shining Sea, 照海 zhào hǎi), ST-44 (Inner Court, 内庭 nèi tíng), and PC-6 (Inner Pass, 内关 nèi guān), needling with drainage.
