True wind stroke

真中风 〔真中風〕zhēn zhòng fēng

Sudden collapse with clouding loss of consciousness, in some cases with deviated eyes and mouth, hemiplegia, stiffness of the tongue impeding speech, and urinary and fecal stoppage, attributed to external wind. See wind stroke.

Biomedical correspondence: cerebrovascular accident.

Medicinal therapy: Course and resolve wind evil. Use variations of Minor Life-Prolonging Decoction (小续命汤 xiǎo xù mìng tāng). For urinary and fecal stoppage in patients with exuberant qì and robust physique, treat by freeing and disinhibiting, using formulas such as Three Transformations Decoction (三化汤 sān huà tāng). If there are no six-channel external signs and no internal obstruction of stool and urine, and only deviated eyes and mouth, inhibited speech, or hemiplegia, the treatment should be to nourish the blood and dispel wind with variations of Large Gentian Decoction (大秦艽汤 dà qín jiāo tāng). Exuberant phlegm-drool congestion and clouding loss of consciousness constitute a block pattern, which must be first treated by opening the orifices, using formulas such as Supreme Jewel Elixir (至宝丹 zhì bǎo dān). Open mouth, limp hands, close eyes, enuresis, snoring, and copious sweating constitute a desertion pattern, which requires large amounts of Center-Rectifying Decoction (理中汤 lǐ zhōng tāng) or Ginseng and Aconite Decoction (参附汤 shēn fù tāng) to support right and stem desertion.

Acumoxatherapy: Base treatment mainly on BL, GB, and TB. Select GB-20 (Wind Pool, 风池 fēng chí), BL-11 (Great Shuttle, 大杼 dà zhù), BL-12 (Wind Gate, 风门 fēng mén), TB-5 (Outer Pass, 外关 wài guān), LI-4 (Union Valley, 合谷 hé gǔ), and LU-7 (Broken Sequence, 列缺 liè quē); needle with drainage. Moxa may be added if necessary.

Point selection according to signs: For urinary and fecal stoppage, add ST-25 (Celestial Pivot, 天枢 tiān shū), TB-6 (Branch Ditch, 支沟 zhī gōu), SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), BL-23 (Kidney Transport, 肾俞 shèn shù), CV-3 (Central Pole, 中极 zhōng jí), and BL-28 (Bladder Transport, 膀胱俞 páng guāng shù); needle with even supplementation and drainage. For deviated eyes and mouth or hemiplegia without obstruction of stool or urine, see hemiplegia and deviated eyes and mouth. For orifice block, add GV-26 (Water Trough, 水沟 shuǐ gōu), GV-20 (Hundred Convergences, 百会 bǎi huì), PC-9 (Central Hub, 中冲 zhōng chōng), LI-4 (Union Valley, 合谷 hé gǔ), LR-2 (Moving Between, 行间 xíng jiān), and ST-40 (Bountiful Bulge, 丰隆 fēng lóng); needle with drainage. For desertion patterns, burn moxa cones at CV-4 (Pass Head, 关元 guān yuán), and CV-6 (Sea of Qì, 气海 qì hǎi), and apply moxibustion on salt or ginger at CV-8 (Spirit Gate Tower, 神阙 shén què) until the sweating ceases, the limbs become warm, and pulse rises.
