- Xī jiǎo (犀角 Rhinocerotis Cornu, rhinoceros horn) (or Shuǐ niú jiǎo (水牛角 Bubali Cornu, water buffalo horn)) 100g
- Zhū shā (朱砂 Cinnabaris , cinnabar) 100g
- Xióng huáng (雄黄 Realgar , realgar) 100g
- Hǔ pò (琥珀 Succinum , amber) 100g
- Dài mào (玳瑁 Eretmochelydis Carapax, hawksbill [turtle] shell) 100g
- Niú huáng (牛黄 Bovis Calculus, bovine bezoar) 50g
- Shè xiāng (麝香 Moschus , musk) 10g
- Bīng piàn (冰片 Borneolum , borneol) 10g
- ān xī xiāng (安息香 Benzoinum , benzoin) 45g
- Jīn bó (金箔 Auri Lamina, gold leaf)
- Yín bó (银箔 Argenti Lamina, silver leaf)
Action: Transform turbidity and opens the orifices; clears the heart and resolves toxin; settles tetany.
Indication: (a) Sudden stroke (i.e., wind stroke) with clouding reversal or summerheat stroke with internal phlegm-damp block impeding speech and causing reversal cold of the limbs; (b) heat entering the pericardium with clouded spirit and inability to speak; (c) acute child fright wind with clouded spirit and tetanic reversal.

Method: The
Dosage: 1 pill 1–2 times a day, taken with a decoction of ginseng or with warm water.
Rationale: Niú huáng is the sovereign agent because acts swiftly to sweep phlegm and open the orifices, as well as to clear the heart and resolve toxin. Shè xiāng, Bīng piàn, and ān xī xiāng repel foulness and transform turbidity, as well as reviving the patient. Thus, they help Niú huáng to open the internal block. Xī jiǎo and Dài mào, which clear the heart and resolve toxin, enhance Niú huáng's fight against the heat evil. Xióng huáng repels foulness and resolves toxin. Zhū shā, Hǔ pò, Jīn bó and Yín bó settle the heart and quiet the spirit. This formula focuses on transforming phlegm and opening the orifices, while ān gōng niú huáng wán (安宫牛黄丸 Peaceful Palace Bovine Bezoar Pill)focuses on clearing the heart and resolving toxin.
