Heat pattern

热证 〔熱證〕 rè zhèng

Any sign or signs indicating the presence of heat. Heat patterns arise when a yáng evil invades the body or when yīn humor becomes insufficient. Thus, heat patterns are caused by a surfeit of yáng or a deficit of yīn, and form repletion heat or vacuity heat patterns.

Patterns

Repletion heat (实热 shí rè): Repletion heat patterns are characterized by red complexion, reddening of the eyes, vigorous heat [effusion], agitation, thirst, desire for cold fluids, hard stool, short voidings of reddish urine, a red or crimson tongue with a yellow fur, and rapid pulse or a rapid large surging pulse.

Medicinal therapy: Repletion heat is treated by clearing, according to the principle that heat is treated with cold. Heat-clearing fire-draining medicinals such as gypsum (Gypsum Fibrosum, 石膏 shí gāo), anemarrhena (Anemarrhenae Rhizoma, 知母 zhī mǔ), coptis (Coptidis Rhizoma, 黄连 huáng lián), and rhubarb (Rhei Radix et Rhizoma, 大黄 dà huáng) are commonly used. Heat-clearing toxin-resolving medicinals are useful in specific cases. Formulas frequently used to treat repletion heat patterns include White Tiger Decoction (白虎汤 bái hǔ tāng) and Heart-Draining Decoction (泻心汤 xiè xīn tāng). In treating vacuity heat, nourishing yīn is of greater importance than clearing heat. Medicinals frequently used include sweet wormwood (Artemisiae Annuae Herba, 青蒿 qīng hāo), black swallowwort (Cynanchi Atrati Radix, 白薇 bái wēi), stellaria (Stellariae Radix, 银柴胡 yín chái hú), lycium root bark (Lycii Cortex, 地骨皮 dì gǔ pí), anemarrhena (Anemarrhenae Rhizoma, 知母 zhī mǔ), and phellodendron (Phellodendri Cortex, 黄柏 huáng bǎi). Sweet Wormwood and Turtle Shell Decoction (青蒿鳖甲汤 qīng hāo biē jiǎ tāng) is a commonly used vacuity-heat–clearing formula. See also supplementing yīn.

Acumoxatherapy: PC-8 (Palace of Toil, 劳宫 láo gōng), HT-8 (Lesser House, 少府 shào fǔ), PC-9 (Central Hub, 中冲 zhōng chōng), HT-9 (Lesser Surge, 少冲 shào chōng), and GV-26 (Water Trough, 水沟 shuǐ gōu) can be used to treat heart fire; LU-10 (Fish Border, 鱼际 yú jì), LU-5 (Cubit Marsh, 尺泽 chǐ zé), LI-11 (Pool at the Bend, 曲池 qū chí), LU-11 (Lesser Shang, 少商 shào shāng), and LI-1 (Shang Yáng, 商阳 shāng yáng) can be selected to treat lung heat; LR-2 (Moving Between, 行间 xíng jiān), GB-43 (Pinched Ravine, 侠溪 xiá xī), and GB-34 (Yáng Mound Spring, 阳陵泉 yáng líng quán) can be selected to treat liver-gallbladder fire; ST-44 (Inner Court, 内庭 nèi tíng), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), LI-4 (Union Valley, 合谷 hé gǔ), and ST-45 (Severe Mouth, 厉兑 lì duì) can be selected to treat stomach fire (heat); ST-25 (Celestial Pivot, 天枢 tiān shū), LI-4 (Union Valley, 合谷 hé gǔ), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), and ST-44 (Inner Court, 内庭 nèi tíng) can be selected to treat large intestine fire (heat); HT-8 (Lesser House, 少府 shào fǔ), ST-39 (Lower Great Hollow, 下巨虚 xià jù xū), and SI-2 (Front Valley, 前谷 qián gǔ) can be selected to treat small intestine fire (heat); CV-3 (Central Pole, 中极 zhōng jí), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo) can be selected to treat exuberant bladder heat; needle with drainage or prick to bleed.

Vacuity heat (虚热 xū rè): Vacuity heat patterns are marked by vexing heat in the five hearts (palms, soles and chest), steaming bone tidal heat, dry throat and mouth, smooth bare red tongue (mirror tongue), and a thin, rapid pulse.

Medicinal therapy: Medicinals frequently used for clearing vacuity heat include sweet wormwood (Artemisiae Annuae Herba, 青蒿 qīng hāo), black swallowwort (Cynanchi Atrati Radix, 白薇 bái wēi), stellaria (Stellariae Radix, 银柴胡 yín chái hú), lycium root bark (Lycii Cortex, 地骨皮 dì gǔ pí), anemarrhena (Anemarrhenae Rhizoma, 知母 zhī mǔ), and phellodendron (Phellodendri Cortex, 黄柏 huáng bǎi). Sweet Wormwood and Turtle Shell Decoction (青蒿鳖甲汤 qīng hāo biē jiǎ tāng) is a formula commonly used for clearing vacuity heat.

Acumoxatherapy: KI-6 (Shining Sea, 照海 zhào hǎi), KI-2 (Blazing Valley, 然谷 rán gǔ), KI-1 (Gushing Spring, 湧泉 yǒng quán), HT-8 (Lesser House, 少府 shào fǔ), and HT-7 (Spirit Gate, 神门 shén mén) can be selected to treat kidney vacuity with fire stirring; ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), SP-3 (Supreme White, 太白 tài bái), and BL-20 (Spleen Transport, 脾俞 pí shù) can be selected to treat spleen-stomach vacuity fire; BL-13 (Lung Transport, 肺俞 fèi shù), BL-43 (Gāo-Huāng Transport, 膏肓俞 gāo huāng shù), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), KI-3 (Great Ravine, 太溪 tài xī), KI-6 (Shining Sea, 照海 zhào hǎi), and KI-2 (Blazing Valley, 然谷 rán gǔ) can be used to treat lung taxation vacuity heat; use both supplementation and drainage to address the vacuity and the heat respectively.
