Genital cold

阴寒 〔陰寒〕 yīn hán

Also yīn cold. A subjective feeling of cold in the anterior yīn (i.e., the external genitals). Genital cold occurs in patterns of vacuity cold of the lower origin, liver channel damp-heat, and damp phlegm pouring downward.

Patterns

Vacuity cold of the lower origin (下元虚冷 xià yuán xū lěng) with congealing of cold qì gives rise to genital cold accompanied by impotence in males and by cold in the abdomen and impaired fertility in women.

Medicinal therapy: Warm the kidney and disperse cold. Use Golden Cabinet Kidney Qì Pill (金匮肾气丸 jīn guì shèn qì wán).

Acumoxatherapy: Base treatment mainly on CV and the three yīn channels of the foot. Select CV-4 (Pass Head, 关元 guān yuán), SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), CV-3 (Central Pole, 中极 zhōng jí), BL-23 (Kidney Transport, 肾俞 shèn shù), CV-6 (Sea of Qì, 气海 qì hǎi), and GV-4 (Life Gate, 命门 mìng mén); needle with supplementation and add moxa.

Liver channel damp-heat (肝经湿热 gān jīng shī rè) can cause coldness of the anterior yīn, testicles, and sacrum and coccyx, aversion to cold and liking for warmth, watery sweating of the genitals, limp wilting anterior yīn (impotence), and dribble after voiding. Genital cold due to liver channel damp-heat was first recorded in Zhāng’s Clear View of Medicine (张氏医通 zhāng shì yī tōng) and is mentioned in some modern texts. The logic of how damp-heat can cause genital cold is obscure.

Medicinal therapy: Clear heat and transform dampness with Bupleurum Dampness-Overcoming Decoction (柴胡胜湿汤 chái hú shèng shī tāng).

Acumoxatherapy: Base treatment mainly on SP, LR, and GB. Select LR-2 (Moving Between, 行间 xíng jiān), GB-43 (Pinched Ravine, 侠溪 xiá xī), GB-34 (Yáng Mound Spring, 阳陵泉 yáng líng quán), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), LI-4 (Union Valley, 合谷 hé gǔ), SI-3 (Back Ravine, 后溪 hòu xī), and KI-7 (Recover Flow, 复溜 fù liū); needle with drainage.

Damp phlegm pouring downward (湿痰下注 shī tán xià zhù) causes genital cold in obese females. Genital cold in women accompanied by itching and continuous vaginal discharge is due to cold-damp.

Medicinal therapy: Dry dampness and abduct phlegm with Two Matured Ingredients Decoction (二陈汤 èr chén tāng) plus atractylodes (Atractylodis Rhizoma, 苍朮 cāng zhú), white atractylodes (Atractylodis Macrocephalae Rhizoma, 白朮 bái zhú), notopterygium (Notopterygii Rhizoma et Radix, 羌活 qiāng huó), and saposhnikovia (Saposhnikoviae Radix, 防风 fáng fēng). Cold-damp with itching and vaginal discharge is treated by inserting Cnidium Seed Powder (蛇床子散 shé chuáng zǐ sǎn) in the vagina.

Acumoxatherapy: Base treatment mainly on CV and ST. Select CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), BL-20 (Spleen Transport, 脾俞 pí shù), CV-6 (Sea of Qì, 气海 qì hǎi), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), and ST-40 (Bountiful Bulge, 丰隆 fēng lóng); needle with drainage. For cold-damp, base treatment mainly on ST and SP. Select ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), and LI-4 (Union Valley, 合谷 hé gǔ); needle with drainage and large amounts of moxa.

Point selection according to signs: For genital itch, add CV-3 (Central Pole, 中极 zhōng jí), BL-34 (Lower Bone-Hole, 下髎 xià liáo), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo). For continuous vaginal discharge, add GB-26 (Girdling Vessel, 带脉 dài mài), BL-30 (White Ring Transport, 白环俞 bái huán shù), CV-6 (Sea of Qì, 气海 qì hǎi), SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), SP-12 (Surging Gate, 冲门 chōng mén), and ST-30 (Qì Thoroughfare, 气冲 qì chōng).
