Blood ejection

吐血 〔吐血〕tù xuè

Ejection of blood through the mouth; vomiting or expectoration of blood (i.e., respiratory tract or digestive tract bleeding); sometimes defined as being associated with neither the sound of retching or of coughing. Blood ejection is attributed to liquor damage, food damage, or taxation fatigue causing exuberant heat in the bowels and viscera, yīn vacuity with effulgent fire, or qì vacuity and spleen cold. The main patterns are accumulated heat in the stomach, intense liver fire, yīn vacuity with effulgent fire, spleen-stomach vacuity cold, and qì vacuity verging on desertion.

Patterns

Accumulated heat in the stomach (胃中积热 wèi zhōng jī rè) causes blood ejection marked by fresh or dark purple blood, distending pain in the stomach duct and abdomen, constipation, yellow tongue fur, and a slippery rapid pulse.

Medicinal therapy: Clear the stomach and drain fire. Use variations of Firewood-Removing Beverage (徙薪饮 xǐ xīn yǐn) or Heart-Draining Decoction (泻心汤 xiè xīn tāng).

Acumoxatherapy: Base treatment mainly on ST, LI, and PC. Select CV-13 (Upper Stomach Duct, 上脘 shàng wǎn), ST-44 (Inner Court, 内庭 nèi tíng), LI-11 (Pool at the Bend, 曲池 qū chí), PC-6 (Inner Pass, 内关 nèi guān), SP-10 (Sea of Blood, 血海 xuè hǎi), ST-37 (Upper Great Hollow, 上巨虚 shàng jù xū), and PC-4 (Cleft Gate, 郄门 xī mén). Needle with drainage.

Intense liver fire (肝火炽盛 gān huǒ chì shèng) blood ejection is characterized by bright red blood with purple stasis, bitter taste in the mouth, rib-side pain, heart vexation, irascibility, red tongue, and rapid stringlike pulse.

Medicinal therapy: Drain the liver and harmonize the stomach with variations of Moutan and Gardenia Free Wanderer Powder (丹栀逍遥散 dān zhī xiāo yáo sǎn).

Acumoxatherapy: Base treatment mainly on ST, LR, and CV. Select ST-34 (Beam Hill, 梁丘 liáng qiū), LR-3 (Supreme Surge, 太冲 tài chōng), LR-2 (Moving Between, 行间 xíng jiān), LR-14 (Cycle Gate, 期门 qī mén), CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), PC-6 (Inner Pass, 内关 nèi guān), BL-18 (Liver Transport, 肝俞 gān shù), and BL-17 (Diaphragm Transport, 膈俞 gé shù). Needle with drainage.

Effulgent yīn vacuity fire (阴虚火旺 yīn xū huǒ wàng) blood ejection is associated with heat effusion, night sweating, tinnitus, insomnia, and a rapid fine pulse.

Medicinal therapy: Enrich yīn and downbear fire; cool the blood and stanch bleeding. Use formulas such as Blood-Cooling Rehmannia Decoction (凉血地黄汤 liáng xuè dì huáng tāng).

Acumoxatherapy: Base treatment on back transport points, KI, and HT. Needle with supplementation at BL-23 (Kidney Transport, 肾俞 shèn shù), KI-3 (Great Ravine, 太溪 tài xī), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo) and with drainage at KI-6 (Shining Sea, 照海 zhào hǎi), KI-2 (Blazing Valley, 然谷 rán gǔ), BL-17 (Diaphragm Transport, 膈俞 gé shù), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), ST-44 (Inner Court, 内庭 nèi tíng), and HT-7 (Spirit Gate, 神门 shén mén). If fire signs are pronounced, prick HT-8 (Lesser Mansion, 少府 shào fǔ) to bleed.

Spleen-stomach vacuity cold (脾胃虚寒 pí wèi xū hán) blood ejection is characterized by ejection of dark, dull, purple blood, fear of cold, cold limbs, and a faint pulse.

Medicinal therapy: Warm the center and stanch bleeding. Use Aconite Center-Rectifying Decoction (附子理中汤 fù zǐ lǐ zhōng tāng).

Acumoxatherapy: Base treatment mainly on CV, ST, BL, and SP. Select CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), BL-20 (Spleen Transport, 脾俞 pí shù), BL-17 (Diaphragm Transport, 膈俞 gé shù), SP-1 (Hidden White, 隐白 yǐn bái), and ST-25 (Celestial Pivot, 天枢 tiān shū). Needle with supplementation and moxa.

Qì vacuity verging on desertion (气虚欲脱 qì xū yù tuō) blood ejection is characterized by exhaustion of essence-spirit, and faint weak soft vacuous pulse.

Medicinal therapy: Boost qì and contain the blood with formulas such as Pure Ginseng Decoction (独参汤 dú shēn tāng).
