CV-13 Upper Stomach Duct

上脘 〔上脘〕 shàng wǎn

Alternate names: 上管 shàng guǎn, Upper Duct; 上纪 shàng jì, Upper Regulator; 胃管 wèi guǎn, Stomach Duct; 胃脘 wèi wǎn, Stomach Duct

Channel: CV, controlling (rèn) vessel

Modern location: An acupoint located on the upper abdomen, 5 cùn superior to the umbilicus on the midline.

Classical location: Directly above Central Stomach Duct (CV-12), five cùn above the navel. From The Golden Mirror of Medicine (医宗金鑑 yī zōng jīn jiàn)

Local anatomy: See CV-12.

Action: Rectifies the spleen and the stomach; transforms phlegm turbidity; courses qì; stabilizes the spirit-mind.

Modern indications: Stomach pain; retching and vomiting; abdominal distension; epilepsy.

Classical indications: Abdominal distension qì fullness; fright palpitation; inability to eat; periodic retching of blood; copious phlegm-drool; running piglet (bēn tún); fulminant heart pain; wind epilepsy febrile disease; dizzy vision; headache and dizziness; generalized heat effusion with or with absence of sweating; sudden turmoil (cholera); accumulations and gatherings; abdominal fullness and rumbling intestines; heat vexation in the heart; jaundice; stomach vacuity distension and fullness.

Needle stimulus: Needling: 0.8‒1.5 cùn perpendicular insertion. Moxa: 5‒7 cones; pole 20‒30 min.

Needle sensation: Distension and numbness extending up and down along the course of the controlling vessel, or extending laterally.

Point groups: Intersection point (jiāo huì xué) of the controlling vessel and the small intestine and stomach channels.
