Qì deserting with blood

气随血脱 〔氣隨血脫〕qì suí xuè tuō

Also blood desertion with qì desertion. A disease pattern chiefly characterized by in a patient suffering massive bleeding, the appearance of qì desertion signs such as scantness of qì and faint breathing, great dripping sweat, indifferent expression, reversal cold of the limbs, and clouded spirit.

Biomedical correspondence: hemorrhagic shock.

Description: Massive bleeding during which the following signs suddenly appear: somber-white complexion, dripping cold sweat, weak faint breathing, reversal cold of the limbs, indifference of spirit-affect or clouded spirit. The tongue is pale and shrunken. The pulse is faint on the verge of expiration or floating and rapid without root (not detectable at the deep level).

Pathogenesis: Bleeding attributable to external injury, gastrointestinal bleeding, and in women, flooding and postpartum bleeding, giving rise to qì desertion and yáng collapse.

Treatment

Medicinal therapy: Treat by boosting qì to stem desertion, which is based on the principle that the first priority in blood desertion is to boost qì. Use formulas such as Pure Ginseng Decoction (独参汤 dú shēn tāng), which both checks vacuity desertion and, by containing the bleed, helps to stanch bleeding.

Acumoxatherapy: Base treatment on GV and CV. For emergency treatment to revive the patient, needle GV-26 (Water Trough, 水沟 shuǐ gōu), GV-20 (Hundred Convergences, 百会 bǎi huì), CV-4 (Pass Head, 关元 guān yuán), GV-25 (White Bone-Hole, 素髎 sù liáo), and KI-1 (Gushing Spring, 湧泉 yǒng quán), needling with supplementation and moxa. Then needle SP-1 (Hidden White, 隐白 yǐn bái), CV-6 (Sea of Qì, 气海 qì hǎi), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), CV-4 (Pass Head, 关元 guān yuán), GV-20 (Hundred Convergences, 百会 bǎi huì), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), applying moxibustion to supplement qì and contain the blood.
