大造丸 〔大造丸〕dà zào wán

Ingredients:

Action: Enriches yīn and downbears fire; boosts essence-blood; supplements the lung and kidney.

Indication: Vacuity taxation and lung-kidney depletion with cough, steaming bone tidal heat effusion, vexing heat in the five hearts, dry throat and mouth, emaciation, and seminal emission, dizziness and tinnitus, night sweating, limp aching lumbus and legs. Tongue: Pale red with scant fur. Pulse: Fine and rapid.

Method: Boil the (cooked) rehmannia with rice to a paste. Grind the other ingredients and blend with paste, and form into pills the size of firmiana seeds. Alternatively, make as honey pills.

Dosage: Take 80–90 pills at a time with brine or a decoction of fresh ginger. Take with wine in winter.

Rationale: Zǐ hé chē supplements qì and nourishes the blood, enriches the kidney and boosts essence. Dì huáng, Mài dōng, and Tiān dōng enrich the lung and kidney; they are used because when sufficiency of kidney water is restored, the fire is naturally restrained. Yīn vacuity commonly leads to qì vacuity, hence Rén shēn is added to boost qì (when qì is exuberant it engenders liquid). Dù zhòng and Niú xī help the Zǐ hé chē and Dì huáng to supplement the liver and kidney and strengthen the sinew and bonesguiban and Huáng bǎi combined with Mài dōng and Tiān dōng clear vacuity heat and eliminate steaming bone. When Dì huáng is boiled with Shā rén and Fú líng, it enriches without clogging stomach qì.
