Alternate name: Fulminant desertion of heart yáng (心阳暴脱 xīn yàng bào tuō)
Key signs: Sudden development of heart palpitation, chest pain, cold sweating, reversal cold of the limbs, clouded spirit, and a faint pulse in patients suffering from heart yáng vacuity.
Description: Dripping cold sweat; reversal cold of the limbs, somber-white complexion; weak faint breathing; in some cases, heart palpitation; acute pain in the heart; clouded spirit; green-blue or purple lips; a pulse that is faint on the verge of expiration. This pattern occurs in patients suffering from heart yáng vacuity and is of sudden onset.
Diseases: Heart palpitation; fearful throbbing; sweating; reversal patterns.
Pathogenesis: This is a severe condition of sudden onset that occurs when heart yáng vacuity reaches its extreme. In addition to heart yáng vacuity, other factors may contribute to this pattern:- sudden damage to heart yáng by cold evil;
- phlegm-stasis obstructing the heart vessels;
- loss of blood or liquid, depriving the heart’s yáng qì of its support.
Analysis of signs
- Heart and chest: Stifling oppression in the heart and chest and heart pain attributable to congealing cold that obstructs the movement of qì and blood.
- Breathing: Weak faint breathing that develops when ancestral qì discharges outward and fails to power respiration.
- Spirit-mind: Clouded spirit arising when the heart spirit is dissipated owing to lack of warmth and nourishment.
- Yáng collapse: Dripping cold sweat, reversal cold of the limbs; somber-white complexion and green-blue lips.
- Complexion: a somber white complexion reflecting the non-movement of blood.
- Tongue: Green-blue or purple, reflecting non-movement of blood.
- Pulse: Faint, on the verge of expiration.

Treatment
