Leg pain

腿痛 〔腿痛〕 tuǐ tòng

Pain in the upper or lower leg. Leg pain is mostly attributable to wind-cold-damp and sometimes to damp-heat; less commonly, it is a sign of a damp phlegm streaming sore.

Patterns

Wind-cold-damp (风寒湿 fēng hán shī) leg pain is due to wind-cold-damp with prevalence of cold and manifests in severe pain (sometimes accompanied by numbness and swelling) and aversion to cold and desire for warmth.

Medicinal therapy: Warm and disperse with White Atractylodes and Aconite Decoction (白朮附子汤 bái zhú fù zǐ tāng).

Acumoxatherapy: Base treatment mainly on CV, SP, ST, and GB. Select GB-20 (Wind Pool, 风池 fēng chí), BL-23 (Kidney Transport, 肾俞 shèn shù), CV-4 (Pass Head, 关元 guān yuán), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo), according to the area affected, add GB-31 (Wind Market, 风巿 fēng shì), ST-32 (Crouching Rabbit, 伏兔 fú tù), BL-40 (Bend Center, 委中 wěi zhōng), and GB-34 (Yáng Mound Spring, 阳陵泉 yáng líng quán), for upper leg, and GB-34 (Yáng Mound Spring, 阳陵泉 yáng líng quán), BL-57 (Mountain Support, 承山 chéng shān), GB-39 (Suspended Bell, 悬钟 xuán zhōng), BL-60 (Kunlun Mountains, 昆仑 kūn lún), and GB-40 (Hill Ruins, 丘墟 qiū xū), for the lower leg; needle with drainage and moxa. For prevalence of cold, warm-needle GV-4 (Life Gate, 命门 mìng mén), and apply moxa at CV-8 (Spirit Gate Tower, 神阙 shén què).

Damp-heat (湿热 shī rè) leg pain is characterized by pain in either the upper or lower leg often with redness, swelling, and heat, and accompanied by reddish urine.

Medicinal therapy: Clear heat and transform dampness with Chinese Angelica Pain-Assuaging Decoction (当归拈痛汤 dāng guī niān tòng tāng). Wind, cold, dampness, or other evils invading in yīn vacuity, yáng vacuity, or kidney vacuity requires treatment that takes account of the vacuity.

Acumoxatherapy: Base treatment mainly on SP, LI, and ST. Select ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), ST-41 (Ravine Divide, 解溪 jiě xī), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), LI-11 (Pool at the Bend, 曲池 qū chí), ST-44 (Inner Court, 内庭 nèi tíng), SP-5 (Shang Hill, 商丘 shāng qiū), and GV-14 (Great Hammer, 大椎 dà zhuī); needle with drainage.
