Pancreas-Clearing Decoction
清胰汤 〔清胰湯〕 qīng yí tāng

Ingredients
- Chái hú (柴胡 Bupleuri Radix, bupleurum [root]) 12–15g
- Huáng qín (黄芩 Scutellariae Radix, scutellaria [root]) 6–9g
- Hú huáng lián (胡黄连 Picrorhizae Rhizoma, picrorhiza [root]) 6–9g
- Xiān máo (仙茅 Curculiginis Rhizoma, curculigo [root]) 6–9g
- Mù xiāng (木香 Aucklandiae Radix, costusroot) 6–9g
- Yán hú suǒ (延胡索 Corydalis Rhizoma, corydalis [rhizome]) 6–9g
- Dà huáng (大黄 Rhei Radix et Rhizoma, rhubarb) 12.15g
- Máng xiāo (芒硝 Natrii Sulfas , mirabilite) 6–9g
Actions: Clears heat and drains fire; rectifies qì and frees the stool.
Indications: Acute pancreatitis with lancinating pain on the upper left abdomen exacerbated by pressure, bitter taste in the mouth, constipation and reddish urine. Tongue: Red with slimy yellow fur. Pulse: Tight, stringlike, and rapid.
Method: Decoct with water.
Rationale: This is a modern formula designed for acute pancreatitis. Chái hú and Mù xiāng regulate qì and course the liver. Bái sháo calms the liver and relieves tension. These agents together treat the liver. Huáng qín and Hú huáng liánclear gallbladder and stomach heat. Dà huáng and Máng xiāo are offensive precipitants that free the interior, allowing the congested heat in the center burner to escape.
Variations:
- Pronounced damp signs and
jaundice : Add Yīn chén (茵陈 Artemisiae Scopariae Herba, virgate wormwood), Zhī zǐ (栀子 Gardeniae Fructus, gardenia [fruit]), and Yú xīng cǎo (鱼腥草 Houttuyniae Herba, houttuynia). - Severe vomiting: Add Zhě shí (赭石 Haematitum , hematite) and Bàn xià (半夏 Pinelliae Rhizoma, pinellia [rhizome]).
- Severe pain: Add Chuān liàn zǐ (川楝子 Toosendan Fructus, toosendan [fruit]) and increase the Yán hú suǒ (延胡索 Corydalis Rhizoma, corydalis [rhizome]).
- Concurrent
food stagnation : Add Lái fú zǐ (莱菔子 Raphani Semen, radish seed),fujianshenqu , Mài yá (麦芽 Hordei Fructus Germinatus, barley sprout), and Shān zhā (山楂 Crataegi Fructus, crataegus [fruit]). Fullness in the chest : Add Hòu pò (厚朴 Magnoliae Officinalis Cortex, officinal magnolia bark) and Zhǐ shí (枳实 Aurantii Fructus Immaturus, unripe bitter orange).- Backache: Add Guā lóu (瓜蒌 Trichosanthis Fructus, trichosanthes [fruit]), Xiè bái (薤白 Allii Macrostemonis Bulbus, long-stamen onion [bulb]), Fáng fēng (防风 Saposhnikoviae Radix, saposhnikovia), and Qín jiāo (秦艽 Gentianae Macrophyllae Radix, large gentian [root]).
- Biliary ascariasis: Bīng láng (槟榔 Arecae Semen, areca [nut]), Shǐ jūn zǐ (使君子 Quisqualis Fructus, quisqualis [fruit]), and Kǔ liàn pí (苦楝皮 Meliae Cortex, chinaberry bark).
- General vacuity with center cold: Omit the Dà huáng (大黄 Rhei Radix et Rhizoma, rhubarb) and Máng xiāo (芒硝 Natrii Sulfas , mirabilite), and add Fù zǐ (附子 Aconiti Radix Lateralis Praeparata, aconite [accessory root]) and Gān jiāng (干姜 Zingiberis Rhizoma, dried ginger).
