Wind-heat invading the lung
风热犯肺 〔風熱犯肺〕fēng rè fàn fèi
Also wind-heat invading the lung (风热袭肺 fēng rè xí fèi). A disease pattern chiefly characterized by cough; thick yellow phlegm; wind-heat exterior signs (pronounced heat effusion with mild aversion to cold).

Description: Cough with expectoration of thick yellow phlegm; nasal congestion and runny nose with turbid nasal mucus; heat effusion with slight aversion to wind or cold; slight thirst; in some cases, painful swollen pharynx; tongue red at the tip with thin yellow fur; a pulse that is floating and rapid.
Diseases: Cough; common cold.
Pathogenesis: Externally contracted wind-heat invading the lung and disrupting the diffusion of lung qì.
Analysis of signs: Wind-heat invading the lung is most common in the springtime.
- Wind-heat invading the lung, causing non-diffusion of lung qì: Cough with thick yellow phlegm, nasal congestion and runny nose with thick yellow nasal mucus, and, in some cases, a painful swollen throat.
- Wind-heat assailing the exterior: Heat effusion with slight aversion to cold. Heat effusion arises because yáng qì rises to the exterior to fight the evil. Aversion to cold arises when wind-heat prevents lung qì from diffusing and obstructs defense yáng, depriving the fleshy exterior of warmth. Wind being mobile and changeable in nature, the sudden onset of the condition.
- Heat: Slight thirst and sore swollen throat.
- Tongue: Red, especially at the tip and margins, with thin yellow fur, reflecting heat.
- Pulse: Floating and rapid. Floating reflects evil qì in the exterior; rapid reflects heat.

- Wind-cold fettering the lung: Wind-heat invading the lung and wind-cold fettering the lung are both external contractions. Wind-heat invading the lung is marked by pronounced heat effusion and mild aversion to cold, a thin yellow tongue fur, and a rapid floating pulse. Wind-cold fettering the lung is characterized by pronounced aversion to cold and mild heat effusion, with a thin white tongue fur, and a tight floating pulse.
- Dryness evil invading the lung: Wind-heat invading the lung is marked by cough with thick yellow phlegm, generalized heat, aversion to cold, a red tongue with yellow fur, and a rapid floating pulse. Dryness evil invading the lung is characterized by dry cough with scant sticky phlegm, dry mouth and throat, aversion to cold with heat effusion, a red tongue with white fur, and a rapid pulse.
- Heat evil congesting the lung: Both wind-heat invading the lung and heat evil congesting the lung have cough as the main sign. However, heat evil congesting the lung is marked by panting as well as cough, and high fever and much more pronounced heat signs.

Clinical sketch: A 29-year-old male three days previously suddenly developed a high fever, cough with phlegm that was difficult to expectorate, periodic pain in the chest, thirst and desire to drink, yellow urine, poor appetite, and disquieted sleep. The tongue fur was slightly yellow. The pulse was floating and rapid. He was diagnosed with wind-heat invading the lung.
