Damp toxin sore

湿毒疮 〔濕毒瘡〕 shī dú chuāng

Also downpour sore. A sore on the lower leg or ankle attributed to wind, dampness, and heat lodged in the skin. Acute forms, attributed to prevalence of damp-heat, begin with flushing of the skin and papules, and with vesicles that itch to exude a yellow discharge. Chronic forms, which are complicated by blood vacuity, are characterized by drying, thickening, and scaling of the skin.

Biomedical correspondence: eczema.

Medicinal therapy: Treat the acute form by clearing heat and disinhibiting dampness. For oral medication use Fish Poison Yam Dampness-Percolating Decoction (萆薢渗湿汤 bì xiè shèn shī tāng) combined with Mysterious Two Pill (二妙丸 èr miào wán). For topical treatment, apply a decoction of phellodendron cold; when the skin has absorbed as much as it can, apply Indigo Powder (青黛散 qīng dài sǎn). Treat the chronic form by nourishing the blood and eliminating wind, using Chinese Angelica Drink (当归饮子 dāng guī yǐn zi) combined with Mysterious Three Powder (三妙散 sān miào sǎn) and applying Indigo Powder (青黛散 qīng dài sǎn) mixed as a paste.

Acumoxatherapy: Base treatment mainly on GV, LI, ST, and SP. Main points: GV-14 (Great Hammer, 大椎 dà zhuī), LI-11 (Pool at the Bend, 曲池 qū chí), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo). For acute cases, needle with drainage to clear heat and disinhibit dampness. For chronic cases, add SP-10 (Sea of Blood, 血海 xuè hǎi), ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ), and HT-7 (Spirit Gate, 神门 shén mén), and needle with supplementation to nourish the blood and dispel wind; also apply the pricking and cupping method.

Point selection according to signs: For pronounced itching, add HT-8 (Lesser Mansion, 少府 shào fǔ) and Itch Reliever (止痒 zhǐ yǎng). For pronounced dampness, add LI-4 (Union Valley, 合谷 hé gǔ) and moxa.
