Water panting

水喘 〔水喘〕shuǐ chuǎn

Panting attributed to water-rheum invading the lung, and occurring when the kidney allows water to collect and the spleen fails to transform dampness and the resulting water qì ascends to the lung impairing depurative downbearing. Signs include qì counterflow rapid panting, oppression and fullness in the chest, abdominal distension, fearful throbbing, and swelling of the head, face, and limbs.

Medicinal therapy: Treatment can be directed at the root or tip according to need. To treat the tip, expel water and disinhibit dampness, and diffuse and downbear lung qì. To treat the root, move the spleen and warm the kidney. If panting precedes distension, focus treatment on the lung; if distension precedes panting, focus treatment on the spleen. Use Minor Black Dragon Decoction (小青龙汤 xiǎo qīng lóng tāng) or lepidium/descurainiae (Lepidii/Descurainiae Semen, 葶苈子 tíng lì zǐ) and black jujube (Ziziphi Fructus Ater, 黑枣 hēi zǎo) to drain the lung. Use Poria, Cinnamon Twig, White Atractylodes, and Licorice Decoction (苓桂朮甘汤 líng guì zhú gān tāng), True Warrior Decoction (真武汤 zhēn wǔ tāng), or Kidney Qì Pill (肾气丸 shèn qì wán) to move the spleen and warm the kidney.

Acumoxatherapy: If panting precedes distension, base treatment mainly on LU, back transport points, and CV. Select Panting Stabilizer (定喘 dìng chuǎn), LU-7 (Broken Sequence, 列缺 liè quē), LU-5 (Cubit Marsh, 尺泽 chǐ zé), BL-13 (Lung Transport, 肺俞 fèi shù), BL-22 (Triple Burner Transport, 三焦俞 sān jiāo shù), CV-6 (Sea of Qì, 气海 qì hǎi), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán), and ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ); needle with drainage. If panting follows distension, base treatment mainly on back transport points, CV, GV, LU, and SP. Select BL-20 (Spleen Transport, 脾俞 pí shù), BL-13 (Lung Transport, 肺俞 fèi shù), BL-23 (Kidney Transport, 肾俞 shèn shù), BL-22 (Triple Burner Transport, 三焦俞 sān jiāo shù), CV-12 (Center Stomach Duct, 中脘 zhōng wǎn), PC-6 (Inner Pass, 内关 nèi guān), CV-6 (Sea of Qì, 气海 qì hǎi), CV-4 (Pass Head, 关元 guān yuán), Panting Stabilizer (定喘 dìng chuǎn), LU-5 (Cubit Marsh, 尺泽 chǐ zé), SP-9 (Yīn Mound Spring, 阴陵泉 yīn líng quán) and ST-36 (Leg Three Lǐ, 足三里 zú sān lǐ); needle with supplementation and add moxa.
