Sudden loss of voice

暴瘖 〔暴瘖〕bào yīn

Also replete metal failing to sound. Loss of voice occurring suddenly, attributed to wind-heat or wind-cold invading the lung and obstructing the pathways of qì and congestion of the lung qì.

Biomedical correspondence: acute laryngitis; spastic aphonia.

Medicinal therapy: Wind-cold is treated by warm acrid effusion and dispersion with Nine-Ingredient Notopterygium Decoction (九味羌活汤 jiǔ wèi qiāng huó tāng). Wind-heat is treated by cool acrid coursing and dispersion with Lonicera and Forsythia Powder (银翘散 yín qiào sǎn) or Throat-Clearing Diaphragm-Disinhibiting Decoction (清咽利膈汤 qīng yān lì gé tāng).

Acumoxatherapy: Base treatment mainly on ST, LU, and LI. For wind-cold, select ST-9 (Man’s Prognosis, 人迎 rén yíng), LU-7 (Broken Sequence, 列缺 liè quē), LI-4 (Union Valley, 合谷 hé gǔ), BL-12 (Wind Gate, 风门 fēng mén), and GB-20 (Wind Pool, 风池 fēng chí). Needle with drainage; use moxa if appropriate. For wind-heat, select ST-9 (Man’s Prognosis, 人迎 rén yíng), LI-11 (Pool at the Bend, 曲池 qū chí), LU-5 (Cubit Marsh, 尺泽 chǐ zé), ST-44 (Inner Court, 内庭 nèi tíng), GV-14 (Great Hammer, 大椎 dà zhuī), and TB-5 (Outer Pass, 外关 wài guān), needling with drainage. If there is high fever, prick the Ten Diffusing Points (十宣 shí xuān) to bleed.
