Dual vacuity of heart qì and yīn

心气阴两虚 〔心氣陰兩虛〕 xīn qì yīn liǎng xū

Insufficiency of heart qì that makes the heartbeat without force, combined with depletion of heart yīn with vacuity heat arising internally. The chief signs are heart palpitation and shortness of breath exacerbated by exertion, insomnia and profuse dreaming, lassitude of spirit and lack of strength, vexing heat in the five hearts, and white face with red cheeks. Other signs include forgetfulness, spontaneous or night sweating, low faint voice, dry mouth and throat, and low fever. The tongue is pale and tender with little liquid. The tongue fur is white, and in some cases, scant. The pulse is fine or fine and rapid.

Analysis: When heart qì and yīn are depleted, the heart is depleted, hence the heart palpitation or fearful throbbing, oppression in the chest and shortness of breath exacerbated by exertion, scantness of breath and lack of strength, and exhaustion of essence-spirit. When the spirit is deprived of nourishment, there is insomnia, profuse dreaming, and forgetfulness. When qì is vacuous, the spirit is deprived of nourishment, hence a frequent desire to sleep. When the fleshy exterior is not kept sound, there is spontaneous sweating and sweating at the slightest exertion. When heart qì is insufficient, its power to move the blood is weakened, and the blood stagnates in the vessels of the heart. When heart yīn is insufficient, vacuity fire harasses the inner body, hence there is heart vexation, heat in the hearts of the palms and soles, and, in severe cases, tidal heat and night sweating. When yīn-liquid is depleted, the throat is dry. When qì vacuity is proment, the tongue is pale and peeled bare; when yīn vacuity is prominent, the tongue is red with little or no fur.

Medicinal therapy: Supplement the heart, supplement qì, and nourish yīn. Use Honey-Fried Licorice Decoction (炙甘草汤 zhì gān cǎo tāng).

Acumoxatherapy: Base treatment mainly on back transport point, HT, PC, and CV. Select BL-15 (Heart Transport, 心俞 xīn shù), CV-14 (Great Tower Gate, 巨阙 jù què), CV-6 (Sea of Qì, 气海 qì hǎi), BL-20 (Spleen Transport, 脾俞 pí shù), PC-6 (Inner Pass, 内关 nèi guān), HT-7 (Spirit Gate, 神门 shén mén), and SP-6 (Three Yīn Intersection, 三阴交 sān yīn jiāo); needle with supplementation.
