True cold and false heat

真寒假热 〔真寒假熱〕zhēn hán jiǎ rè

Any disease pattern presenting with signs of both heat and cold, in which the cold signs represent its essential nature, whereas the heat signs are deceptive. This accords with the principle that extreme cold resembles heat. True cold and false heat is identified when reversal cold of the limbs, diarrhea containing untransformed food, or faint pulse verging on expiration are found occurring simultaneously with agitation, no aversion to cold, thirst with desire for warm fluids, upbearing fire flush, or sore pharynx. Here, the cold signs are caused by exuberant internal yīn cold, whereas the false heat signs (agitation, upbearing fire flush, and sore pharynx) are attributable to vacuous yáng floating upward. The principle of treatment applied in such cases is treating heat with heat.
